Position Overview: The case manager must be an honorably discharged veteran to work in the Texas Veterans Commission Project.  This position will oversee the day-to-day activities, services, and auxiliary resources related to Meals On Wheels clients and their families that are veterans. This position will work close with all other departments of Meals On Wheels with regard to client intakes, assessments, program on-boarding, monitoring of services, and the initiation of new program. This position requires driving your personal vehicle for the client home visits and the delivery of client meals and/or agency business.

Paid As: Hourly, Non-Exempt Full-Time Employee

Hours of Operation: 8:00 AM – 4:30 PM Monday – Thursday, 8:00 AM – 3:30 PM Friday

Reports To: Vice President Client Services

 AGENCY EXPECTATION OF EMPLOYEE

  • Maintain confidentiality of client, donor, volunteer, staff and board information
  • Support the mission, vision and goals of Meals On Wheels, Inc. of Tarrant County
  • Represent Meals On Wheels in a professional manner at all times
  • Must have dependable transportation, valid driver license and personal vehicle insurance as required by the State of Texas
  • Required to pass criminal background check and have a free and clear MVR
  • Assist with meal delivery when volunteer callouts make it necessary for staff to deliver
  • Contribute to other projects when necessary

 ESSENTIAL JOB TASKS

  • Support the overall purpose and vision of the Texas Veterans Commission (TVC) Grant
  • Understand the specific tasks as outlined within the contract and work to ensure success with implementation of the TVC Grant
  • Must possess self-management skills such as initiative, organization and independence.
  • Provide direct case management of services to approximately 150 agency clients who are veterans, a veteran’s spouse, or a veteran’s dependent
  • Willing to work with clients throughout Tarrant County
  • Work positively in a fast-paced environment with the understanding that changes occur frequently and the staff must be able to manage change in a professional manner
  • Communicate regularly with the Vice President Client Services to update on progresses, challenges, and successes
  • Work directly with multiple community resources in a professional manner to include other TVC funded agencies
  • Must be able to meet hard deadlines with accurate reporting and maintain accurate data in the client database
  • Complete intakes, maintain a personal caseload, data entry and reports, handle client emergencies, deliver meals, conduct home visits to meet client needs and make re-certifications at a minimum of 3 times in a year with other visits dependent upon client needs
  • Other duties as assigned by the Vice President Client Services, the Texas Veterans Commission Grant, or required by other agency programs or services

 QUALIFICATIONS

  • Four-year university degree in Social Work, Human Services, Psychology, Sociology, or related field
  • Honorably discharged veteran of the US military/armed services
  • Previous work with TVC funded agencies preferred

REQUIREMENTS

  • Excellent customer services skill with emphasis on respectful and tactful communication
  • Proficient written and oral communication skills
  • Communicate with a clear, concise and empathetic manner with clients, donors, volunteers, staff, and board members
  • Personal qualities of integrity, credibility, discretion about confidential matters and possess a strong work ethic
  • The position includes strenuous activity when picking up supplies and equipment from partnering agencies and community services
  • Work often requires sustained periods of loading and lifting of heavy items in both temperate and uncontrolled environments and all types of weather conditions
  • Must be able to lift, load and unload 40 lbs
  • Physically able to climb stairs, repeatedly get in and out of vehicle, reach, stretch, bend and sit or stand for long periods of time

EXPERIENCE

  • 2 years prior case management workload
  • Knowledge of computer operation and software products include Microsoft Office Suite
